If the Rebuild Database option doesn’t resolve the issue, access Safe Mode and select Reset PS5 (Reinstall System Software).

With the cover removed, look for black rings on the disc drive, including a circle of black plastic film. You can remove this film, or push through it to reach the screw it is covering. (Newer consoles may not have this plastic cover.)
Using a #1 Phillips or a cross-hatch screwdriver, turn the screw CLOCKWISE until the full disc is almost completely visible.
Gently remove the disc. Do not forcibly pull the disc from the disc drive.
Replace the bottom cover by positioning it slightly away (about 2cm or 3/4 inch) from the top edge and sliding it back into place. There is an audible click when the cover is secured.
Reconnect all cables and turn on the console.
Check the disc for scratches and dirt
Clean a scratched or dirty disc using a lint-free cleaning cloth made specifically for cleaning Blu-ray Discs or DVDs.
Clean the disc by wiping from the center towards the edge of the disc.
